Service Evaluation of the Transfer and Discharge of Patients from St Richard’s Hospital to Community Hospitals to Home

  Mar 2012   The aim of this study was to investigate patients’ experiences and their feelings about transfer from St Richard’s Hospital to a community hospital and subsequent discharge to home in order to promote good practice and make improvements for patients.

Discharge Report

Service Evaluation of the Discharge of Short Stay Patients from St Richard’s and Worthing Hospitals.

 

 

Jan 2011

  The aim of this study was to collect information from patients about their experiences and feelings of their discharge from hospital to home with a view to promoting good practice and making improvements for patients. Some ninety four patients took part, of whom the vast majority were satisfied with their discharge experience but almost a fifth were not. Nine percent of patients discharged from hospital to their homes were re-admitted, the majority within one week for a variety of medical reasons…

 

Advocacy Report   Dec 2010   This report into the state of Advocacy services in West Sussex was commissioned by West Sussex LINk and prepared by the Social Science Policy and Research Centre at the University of Brighton.Following approval from the LINk Stewardship Group, the report is being circulated to stakeholders and partner agencies, who have been invited to comment on the report contents and its recommendations.

 

Sexual Health Mystery Shopping Report   Nov 2010   This report from West Sussex Youth Cabinet & West Sussex LINk is the result of a 'mystery shopping' exercise and records the experiences of young people when they accessed the sexual health service provided across West Sussex by Western Sussex Hospitals NHS Trust. It has been sent, along with action plan to address the areas highlighted, to the Commissioner- Sexual Health and Emotional Wellbeing, NHS West Sussex.

 

Listening to Black and Minority Ethnic | (BME)Communities   Sept 2010   The LINk has begun to liaise with BME communities, groups and support workers across West Sussex in order to build a county picture of BME engagement and the issues that have emerged but this work is in its early stages. This report therefore presents a brief overview.

 

Health and Social Care Needs of Gypsy and TravellerFamilies andcommunities in WestSussex   Sept 2010   The LINk has been working with Friends, Families and Travellers (FFT) to address the issues facing Traveller communities across West Sussex.  FFT is a well established organisation working to address such issues and together we have sought to identify and address some of the barriers which are preventing these families from accessing support for their health and social care needs.

 

Disabled Facilities Grants   August 2010   Information from the county’s District and Borough Councils highlights delays in the length of time taken to get grants from the different authorities - some up to 10 months - and will serve as a useful benchmark for future reference;

 

WS LINk & Youth Cabinet Survey   Aug 2010   This report is a result of the partnership between the LINk and the West Sussex Youth Cabinet. It was compiled on the basis of a series of health and well-being related questions circulated to young people across West Sussex as part of the Youth Cabinet's election process in March 2010. Just under 2,500 young people responded with their views on issues affecting the general well-being of young people in the county.

 

Nutrition Report

Report from Brighton and Hove LINk and
West Sussex LINk on Nutrition in the Royal
Sussex County Hospital (Brighton) and
Princess Royal Hospital, Haywards Heath.

  June 2010   East Sussex Health Overview and Scrutiny Committee (HOSC) commissioned
East Sussex, Brighton and Hove (B&H) and later West Sussex LINks to feed into
their nutrition, hydration and assistance with eating in Sussex Hospitals. The
rationale was to compare and contrast data throughout Sussex, highlighting good
practice and identifying areas of improvement.

 

WS LINk Annual Report (2009-2010)   June 2010   This Annual Report covers the period 1st April 2009 to 31st March 2010. Although this is the LINk’s second Annual Report, because neither the Stewardship Group nor the Host Team were fully up and running until April 2009, it is effectively a record of the first year of the West Sussex LINk’s life.

 

West Sussex LINk response to Mental HealthConsultation   June 2010   This report is the LINk response to the public consultation into Mental Health Services in West Sussex. This is just part of the feedback from the public which, along with notes of the public meetings, have been collated and sent to an independent analyst. 

 

Enter & View Report: East Surrey Hospital   May 2010   This document reports on the visit made by West Sussex and Surrey LINks to the East Surrey Hospital, Redhill, in response to public and patient concern at the findings of the Care Quality Commission Inspection Report:The prevention and control of infections; Surrey andSussexHealthcare NHS Trust, 3 March 2010. The purpose of the visit was to provide a lay perspective on conditions being experienced by patients, especially with regard to cleanliness and hygiene, on two wards in the hospital.

 

Annual Report (2008 - 2009)   Aug 2009  

LINks are required to produce annual reports for the Secretary of State for Health and the Care Quality Commission. Copies have also been sent to West Sussex County Council, the West Sussex Primary Care Trust, the Strategic Health Authority and Overview and Scrutiny Committees. 

The report covers the period 1st April 2008 to 31st March 2009, prior to the establishment of the current Stewardship Group, when the foundations were being laid for the West Sussex LINk.

 

Patient and Public Engagement aroundFit for the Future Report   Aug 2009   The LINk was asked by NHS West Sussex to scrutinise their patient and public engagement around the Fit for the Future commissioning intentions review. This resulting report was sent to the commissioners with recommendations for improvements in the future.
