The aim of this study was to collect information from patients about their experiences and feelings of their discharge from hospital to home with a view to promoting good practice and making improvements for patients. Some ninety four patients took part, of whom the vast majority were satisfied with their discharge experience but almost a fifth were not. Nine percent of patients discharged from hospital to their homes were re-admitted, the majority within one week for a variety of medical reasons… |

It was compiled on the basis of a series of health and well-being related questions circulated to young people across West Sussex as part of the Youth Cabinet's election process in March 2010. Just under 2,500 young people responded with their views on issues affecting the general well-being of young people in the county. |

The purpose of the visit was to provide a lay perspective on conditions being experienced by patients, especially with regard to cleanliness and hygiene, on two wards in the hospital. |

The report covers the period 1st April 2008 to 31st March 2009, prior to the establishment of the current Stewardship Group, when the foundations were being laid for the West Sussex LINk.
