The West Sussex Local Involvement Network (LINk) is “a way for youto have your say in how your health and social care services aredelivered”

One way to have your say is to tell us about A RECENT TIME WHEN YOU HAVE USED A HEALTH OR SOCIAL CARE SERVICE in West Sussex. This type of information helps the LINk to identify real issues and examples of shortcomings in service levels that need to be addressed.  

If you would like to help with this, we have designed a short 'Have your Say' form that you can complete on-line. It is quick and easy to complete and you can be as brief as you like - although the more information you include the more helpful the data tends to be.

The structured format should help you to recall and record your experience in a way that will enable the LINk to clearly identify issues and trends in service levels - both good and bad.

ACCESS TO THE SERVICE: this could include your comments about transport, parking, distance to travel, cost of travel or, if you received treatment at home, about how easy it was to get the service at home.
Did you find it easy to accesss the service you needed?
   
 

SECTION 5:

 
WAITING TIME: this could include comments about how long it took to get the appointment (waiting list), how long it took to be seen on the day, or about being kept waiting on the phone at home.
Did you have to wait long for the appointment or to be seen on the day?

DISABILITY: this could include comments about physical access, inability to hear announcements or read information through sight or hearing impairments, or because of difficulties reading or speaking English.
